Member/Guest Agreement Forms

When do I need to do this?

All Members, Class Attendees, and Guests are required to complete this agreement. It only needs to be completed once, before your first class or first instance of membership.

Do children under 16 need to complete this?

If someone under 16 is attending a class (or hanging out in the shop for an extended period of time), their parent or guardian must sign the agreement for them.

Do I need to print and sign it ahead of time?

Only if you are the guardian of a child and you will not be there in-person with them when they come to the shop.

• This is a common situation for Girl Scout troops, high school robotics teams, etc.

Otherwise, you do not have to print the paperwork yourself.

The Agreement includes:

• Section 1: Personal Information – your contact information

• Section 2: Release, Indemnity and Assumption of Risk – a legal document, please read thoroughly (If the attendee is a minor, a parent or guardian must complete this and enter the minor’s name in part 6.)
